Residential Service Associate – Mental Health Group Homes

Conexio Care serves as one of Delaware’s largest non‑profits, collaborating with government, community, corporate and other philanthropic partners to maximize services for the state’s most vulnerable. The Program Ambassador is a member of the clinical team who assists clients and guests in gaining access to safe, timely, and quality services.

Position Statement

This member of the multidisciplinary team has the responsibility to carry out rehabilitation and support functions and assist in treatment. Provides individual support, substance abuse services, family support, services linkage, daily living skills support, assistance with self‑administration of medication, employment support, social‑leisure support and other activities as assigned under the supervision of a mental health professional.

Responsibilities
  • Participate in treatment planning and case conferences with other staff of the community support program and with other agencies.
  • Complete all assigned tasks from the daily schedule, including maintaining a therapeutic alliance with clients, referring and linking clients to all needed services provided outside the agency, and following up to ensure those services are delivered.
  • Coordinate the provision of medical and vocational services.
  • Coordinate overall independent living assistance services and work with community agencies to develop needed resources such as housing, employment options, and income assistance.
  • Provide support and consultation with consumers’ families.
  • Provide in‑home services including ADL training, medication assistance and training, social support and home management assistance.
  • Provide supportive counseling.
  • Document and keep other staff, clinical coordinators and Program Director informed of all changes in the consumer’s mental or social status.
  • Carry primary or secondary cell phone as assigned and respond to after‑hour and emergency calls in accordance with program directives.
  • Provide education and advocate for persons served in the community.
  • Perform other duties as requested or assigned, verbally or in writing.
Education & Experience
  • High School Diploma.
  • At least two years of experience supporting individuals with severe and persistent mental illness.
Skills and Training
  • CPR/First Aid Certification.
  • Valid Driver’s License with clean driving record.
  • Satisfactory Criminal Background Investigative Report.
  • Crisis Prevention and Intervention Training.
  • LLAM Certification.
  • Training in Defensive Driving.
  • Training in Infection Control.

  • Training in Working with Persons with Serious Mental Illness.
  • Training in PM‑46 and how to identify and report abuse, neglect, mistreatment and misappropriation of property.
  • Training in 45 and 42 CFR, Rules and Regulations that safeguard a person’s privacy.
  • Training about Agency policies and procedures on both a daily and emergency basis.
  • Training in Fire Safety and Evacuation.
  • Training in Safe Food Handling.
  • Training about local resources in the community.
Network Functions
  • Maintain and abide by OSHA requirements and guidelines.
  • Observe all Fire and Safety Policies and Procedures.
  • Observe all Personnel Policies and Procedures.
  • Observe all Agency’s Policies and Procedures.
  • Observe all NJDOH‑DMHAS, CARF and NJSAMS Regulations.
  • Observe all HIPAA Policies and Procedures and maintain confidentiality.
  • Complete and return all paperwork distributed by the Human Resource Department.
  • Attend all required and scheduled trainings and meetings.
  • Always maintain a professional demeanor and exercise good professional judgment in all areas of employment duties.
  • Document time worked accurately.
Equipment and Machines
  • Telephone, cellular telephone, copier, fax, computer, calculator.
Computer Software Applications
  • Computer literacy in MS Office products: Word, Excel, Outlook, PowerPoint.
  • Agency Electronic Medical Record and other software programs.
Physical Demands
  • Individual must be able to meet physical requirements of the job: sitting at a meeting table or desk, reaching, lifting, bending, standing, stooping, twisting, climbing steps, driving a vehicle.
