Qualifications

• Active RN license in Massachusetts
• Minimum 2 years RN experience in residential/community setting preferred

• Reliable transportation and travel between sites within greater Springfield area

Responsibilities

• Provide clinical oversight to residential staff supporting individuals with ID/DD
• Collaborate with Lead RN, Clinical Director, PBS clinicians, and team
• Participate in team meetings to review care plans and outcomes
• Serve as MAP consultant per DPH policies and manage health assessments
• Support safe hospital discharges and follow-up care
• Provide MAP and health-related training to staff
• Audit MAP processes and ensure DDS/DPH regulatory compliance
• Maintain accurate medical records and document health changes
• Attend ISP and MAP meetings and support quality improvement
