Responsibilities

• Assess and monitor client needs.
• Develop service plans for clients.
• Connect clients to community services and resources.
• Advocate for clients when necessary.
• Assist with program enrollment and orientation.
• Coordinate client medical appointments, transportation, housing, money management.
• Develop and assist with medication management plans.
• Obtain and maintain entitlements for clients.
• Maintain accurate and complete documentation.
• Develop and coordinate transition plans and program discharge.
