Qualifications

Responsibilities

• Coordinate and monitor clients' health care services and care coordination.
• Assist in discharge planning for clients with medical needs.
• Develop and update Nursing Care Plans and Health Care Summaries.
• Liaise between external health providers and the IDT.
• Monitor and stock medical supplies and maintain medical records.
• Provide direct care and first aid to clients as needed.
• Plan and present infectious disease training for staff.
• Promote professional relationships with clients, families, and coworkers.
