Job summary

The Colorado Department of Human Services (CDHS) is hiring for a Mental Health Transition Living Homes team member in Colorado. You will provide direct client care in a 24/7 residential setting, assist with ADLs, and support development of social and life skills, under supervision.

This full-time, in-person role requires a High school diploma or GED, and may require QMAP certification within 6 months. Rostered shifts include day, evening, and overnight hours, including weekends.

Qualifications

• 1 year of direct care experience with clients suffering from SMI.
• Experience passing medications.
• Experience with de-escalation techniques.
• QMAP certification within 6 months of hire.

Responsibilities

• Provide direct client care with ADL support and life skills development.
• Ensure safety and supervise clients in a 24/7 residential setting.
• Administer medications per MAR and document responses.
• Monitor client activities, document shift notes and logs.
• Assist with transportation and client orientation to the program.
