Job summary

Thresholds is seeking a Community Support Specialist for the Rogers Park area to provide direct services including individual and group counseling, crisis intervention, and vocational support. The role emphasizes relapse prevention planning and psychosocial rehabilitation within a residential setting.

The position requires a high school diploma or GED with experience or a Bachelor’s degree in Psychology or Social Work, and daily access to a vehicle with a valid license to support transportation

Qualifications

• High school diploma or GED with 5 years supervised job-related experience or
• Bachelor’s degree in Psychology, Social Work, or related field

Responsibilities

• Develops and reviews members’ care plans by identifying goals, objectives, and services needed.
• Teaches independent living skills (housekeeping, meals, coping strategies).
• Provides support to members with collateral social service agencies and the community.
• Documents services in an Electronic Health Record in a timely manner.
