Job summary

Advocates is seeking a Licensed Practical Nurse to provide nursing care in multiple residential programs in the Metro West area. You will perform medical management, coordinate with families and healthcare providers, and supervise direct care staff under the Residential Program Nurse.

The role requires MA LPN licensure with about 1 year in a community healthcare setting, strong communication skills, a valid driver's license, and a patient-centered approach consistent with Advocates' inclusive

Qualifications

Massachusetts LPN license required with 1 year of community healthcare experience.
Strong written and oral communication with residents, families, providers, and staff.
Excellent written and verbal communication skills.
Must have a valid driver's license and access to a vehicle for client transportation.
Diversity awareness and competence in serving diverse populations.

Responsibilities

Assist residents with medical management including physicals, referrals, prescriptions, appointments and treatments, and provide emergency first aid.
Coordinate care with residents, families, physicians, and site teams; note changes in status or medication.
Oversee medication management documentation per agency and funder regulations.
Supervise direct care staff under the Residential Program Nurse.
Monitor care continuity and quality, proposing improvements as needed.
Participate in staff and service planning meetings, and resident/family meetings.
