Job summary

Advocates is seeking a Licensed Practical Nurse to provide nursing services under the direction of the Director of Health Services. The role supports multiple residential programs in the Metro West area and requires MA licensure with at least 1 year of community healthcare experience.

The schedule includes first shift with rotating start times, and starting rate is $33.00 per hour. A valid driver’s license and reliable vehicle are preferred to transport clients when needed.

Qualifications

• Massachusetts LPN with 1 year experience in community healthcare.
• Strong oral and written communication with residents, families, providers, and staff.
• Excellent written and verbal communication.
• Must have a valid driver’s license and access to a vehicle for transporting clients.
• Desire to work with diverse populations.

Responsibilities

• Assist residents with all aspects of medical management including physicals, referrals, prescriptions, appointments and treatments.
• Coordinate with residents, family members, physicians, other medical providers, and site team regarding medical care and changes in status, treatment or medication.
• Implement, oversee, document and supervise medication management in accordance with regulations.
• Oversee direct care staff under the Residential Program Nurse.
• Monitor and evaluate the continuity and level of care, proposing quality improvements.
• Participate in staff meetings, trainings, service planning meetings, and family meetings.
