Job summary

Qualifications

• Associate degree in behavioral health or similar field is required.
• 2–4 years related experience or equivalent combination required.
• Knowledge of mental illness, substance use, and eligibility for state entitlement programs preferred.
• Must have current driver’s license.

Responsibilities

• Provide outreach to individuals with mental illness or SUD who are homeless or at risk of homelessness and coordinate with other agency staff.
• Assist clients with budgeting and rental applications to secure housing.
• Verify income and assist with benefit or employment applications as needed.
• Conduct apartment inspections and ensure units are clean and orderly between residents.
• Maintain documentation, progress notes, and releases of information in accordance with requirements.
• Participate in on-call duties and coordinate with referral sources to support client housing stability.
