Job summary

RiseWell Community Services is seeking a full-time Case Manager to join our Residential Housing Team in Brentwood/Medford, NY. You will provide case management services, housing support, and assistance to residents with overcoming barriers associated with their health conditions so they may live successfully in on-site apartments.

RiseWell’s Residential Services offer comprehensive support including medication monitoring, skill-building, symptom management, socialization, transportation

Qualifications

BA in Human Services or equivalent.
Experience working with people with disabilities or older adults a plus.
Clean, valid NY State Driver’s License.
Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Responsibilities

Provide case management, housing support, and assistance to residents.
Provide information, training, hands-on assistance, and skill-building to individuals served.
