Job summary

Services for the UnderServed is seeking a Residential Habilitation Specialist to develop plans and goals under each person’s ISP, promoting independence in the community.

You will train staff, document data monthly, write notes, and participate in Semi-Annual and Annual ISP reviews with written progress reports.

Qualifications

• Bachelor’s degree in a human services field.
• At least 2 years experience collaborating with people with intellectual and developmental disabilities.
• Strong written communication and data documentation skills.
• Proficient with Microsoft Word, Excel, and PowerPoint.
• Experience writing formal goals/outcomes and staff action plans preferred.
• SCIP-R training is a plus.

Responsibilities

• Develop Residential Habilitation Plans aligned with each person’s ISP.
• Set formal goals to help individuals acquire skills to meet outcomes.
• Provide in-service training to staff and data collection routines.
• Write monthly Residential Habilitation notes for each individual.
• Attend Semi-Annual and Annual ISP reviews and provide written progress reports.

• Develop formal goals as needed to assist individuals in acquiring skills necessary to meet their valued outcomes
• Provide in-service training to staff on how to implement plans, collect necessary data, and turn over data sheets on a monthly basis
• Write a monthly Residential Habilitation note for each individual
• Attend Semi-Annual and Annual ISP reviews and provide a written report for Semi-Annual and Annual ISP reviews regarding the Residential Habilitation being provided to each individual and the progress being made

Min USD $58,500.00/Yr.

Max USD $59,800.00/Yr.

Position Overview

The Residential Habilitation Specialist ensures that necessary residential habilitation is provided in an effort to assist in maximizing independence and integration in the community for individuals living within an Individualized Residential Alternative.

Qualifications

REQUIRED EDUCATION AND EXPERIENCE

• At least a Bachelor’s Degree in a human services field
• At least 2-years experience collaborating with people with intellectual and developmental disabilities

Preferred Qualifications & Skills

• Able to use Microsoft Word; Microsoft Excel; PowerPoint with proficiency
• Proficient writing and organizational skills
• Excellent time management to complete multiple tasks simultaneously
• Experience writing formal valued outcomes/goals and/or Staff Action Plans preferred
• SCIP-R trained
