Qualifications

• Bachelor's degree in mental health, psychology, sociology, social work, human services, or related field.
• Alternatively, a combination of college coursework plus four years of related experience.
• High school diploma or GED with four years of related work or life experience.

Responsibilities

• Support consumers in achieving personal care goals and addressing ongoing challenges.
• Educate consumers on medications, track logs, and conduct monthly audits.
• Coordinate services based on individual needs.
• Transports consumers using the company van as needed.
• Deliver high-quality care in line with IRP.
• Document services within required timeframes using electronic health records.
