Qualifications

• Must pass required certifications in the allotted time set forth by the agency.
• Must pass a criminal background check and driving record check.
• Valid driver's license and reliable transportation are required.
• High school diploma or equivalent is required.

Responsibilities

• Supervise and transport clients during assigned hours, including appointments and activities.
• Support clients in gaining new skills aligned with Individual Service Plans.
• Be a positive role model to help clients achieve quality of life and safety.
• Administer medications and complete timely documentation (progress notes, summaries).
