Qualifications

• High School Diploma or GED required.
• Valid Driver’s License with acceptable driving record required.
• Reliable, eager to learn, and open to new opportunities.

Responsibilities

• Assist in developing individualized, person-centered service and support plans and implement them.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Help individuals access transportation and community resources as needed.
• Implement activities that promote community inclusion, relationship building, and personal growth.
• Maintain a safe, clean, and supportive environment focused on health and well-being.
• Review, complete, and maintain documentation in an accurate and timely manner.
