Job summary

Compass Health Network seeks a Community Support Specialist - Residential to partner with adults on behavioral health and substance use recovery goals. You’ll provide personalized, community-based support in homes, offices, and in the community, with driving as an essential function.

You’ll help clients set wellness goals, access services, and build independent living skills while collaborating with a care team. This is based at Adapt of Missouri, a Compass Health Network subsidiary.

Qualifications

• High School/GED required; associate or bachelor’s in a human services field preferred.
• Qualified CSS requirements include experience or related education in behavioral health support.

Responsibilities

• Partner with clients and families to support recovery and resiliency goals.
• Provide holistic services addressing physical, mental, and social wellness.
• Assist with Wellness Plans and access services like housing, healthcare, and community resources.
• Offer education and skill-building to promote independence and daily living.
• Coordinate with integrated health team and community partners to ensure coordinated care.
• Support transitions such as hospitalization and discharge planning with ongoing communication.
• Complete timely documentation and uphold agency service standards.
