Job summary

Compass Health Network’s Adapt program in St. Louis seeks a Community Support Specialist - Residential to help adults pursue recovery and wellness. You will provide personalized, community-based support in clients’ homes, offices, and in the community, coordinating with a multidisciplinary team.

You will help clients set recovery goals, develop daily living skills, access services, and build supportive relationships while driving to meet clients. This role is full-time and benefits eligible.

Qualifications

• High School or GED is required.
• A social services related degree is preferred.
• Valid driver’s license required.ク

Responsibilities

• Support recovery goals using a motivational, strengths-based approach.
• Provide holistic services addressing wellness dimensions (physical, mental, social).
• Assist clients in developing and achieving Wellness Plans and accessing services.
• Offer education and skill-building for independence in daily living and symptom management.
• Coordinate with integrated health team members and community partners for coordinated care.
• Provide transportation and other direct supports; driving is essential.
• Document timely and uphold service standards.
