Job summary

Signature Health in Mentor, OH seeks a Specialist, Residential Care to provide direct care-support to residents in a residential facility. You will work closely with the clinical team to support daily tasks, assist with meals and activities, and help transport residents to appointments while documenting medication self-administration in the electronic health record.

The role requires a High School Diploma or equivalent, 1+ year in a residential/healthcare setting, an Ohio driver’s license, and

Qualifications

• High School Diploma or equivalent required.
• 1+ year experience in a residential facility or health care setting preferred or required.
• Valid unencumbered Ohio driver’s license and proof of driver’s insurance required.
• AHA Basic Life Support (BLS) certification required.
• Knowledge or experience in behavioral health setting preferred.

Responsibilities

• Provide direct care-support to residents in a residential facility.
• Assist in meal preparation and serving; engage residents in social/recreational activities.
• Transport/escort residents to appointments and activities; document medication self-administration in the EHR.
• Document each shift in the log and share verbal reports at shift change.
• Participate in team meetings with the clinical team to support treatment plans.
