Job summary

ElevaCare is seeking a compassionate Behavioral Health Practitioner to support residents at Unity House Intensive Residential Treatment Facility in Worthington, MN. You will implement treatment plans, advocate for residents, facilitate group activities, assist with appointments and transportation, administer medications, support meals, and document progress.

Qualifications

• Bachelor's degree path: Bachelor's in behavioral sciences or related field, 2,000 hours clinically supervised experience, completion of required training, and practicum/internship experience.
• Advanced experience path: 4,000 hours experience working with mental health, substance use, TBI, or developmental disability clients, completion of required training, or ongoing supervision until requirements are met.
• Graduate level: Master's or other graduate degree in behavioral sciences or related field, or enrollment as a graduate student assigned to ElevaCare for clinical training.

Responsibilities

• Implement and enforce residential program policies and procedures
• Collaborate with team members to develop, review, and carry out individualized treatment plans
• Provide direct support to residents: counseling, advocacy, appointments, transportation, meals, medication administration, and group activities
• Maintain accurate and timely documentation of treatment, progress, and evaluations
• Participate in supervision, staff meetings, and trainings
• Maintain relationships with referring agencies, guardians, and programs involved in residents’ lives
• Support crisis intervention as needed
