Community Health Resources (CHR) is seeking a Part-time Residential Aide for Heartwood Home in Willimantic, CT. This role supports residents in a structured recovery residence, with duties including medication monitoring, life-skills coaching, and group activities.

Responsibilities

• Maintains physical environment and program standards to CHR, DMHAS, JCAHO and other licensing standards.
• Monitors medications of all residents in accordance with the Adult residential medication policy and procedure.
• Transmits information to staff regarding day’s events using appropriate communication methods/tools.
• Teaches residents in rehab areas such as nutrition, healthy living, meal preparation, hygiene, laundry, budgeting, and basic home care skills.
• Assists/Manages administrative functions and tasks as directed by supervisor.
• Prepares and facilitates educational and skill-building groups and one-on-one interventions as directed by the treatment plan.
• Collaborates and coordinates care with providers and families as needed.
• Addresses residents’ medical and psychiatric needs and maintains documentation per standards.
• Participates in staff meetings, trainings, and other meetings as assigned.

JOB TITLE: Residential Aide

EMPLOYMENT TYPE: Part-time, 16 hours/wk

SCHEDULED HOURS: Thursday, Friday 4pm-12:30am

ABOUT THE PROGRAM: Heartwood Home

Heartwood Home is a 12 bed 3.1 level residential program for men aged 18 and older located in Willimantic CT. Heartwood Home provides a structured recovery residence environment, staffed 24 hours a day, which provides sufficient stability to prevent or minimize relapse or continued use. Individuals have an opportunity to develop and practice their interpersonal and group living skills, strengthen their recovery skills, reintegrate into the community and family, and begin or resume employment or academic pursuits. Clients with co-occurring disorders and on Methadone maintenance are accepted. The length of stay is typically between 30-90 days. The range in duration is individualized and based on medical necessity and clinical progress.

ABOUT THE POSITION: Residential Aide

Duties & Responsibilities

• Maintains physical environment and program standards to CHR, DMHAS, JCAHO and any other licensing standard


• Monitors medications of all residents in accordance with the Adult residential medication policy and procedure.


• Effectively utilizes appropriate communication methods/tools to transfer information to other staff regarding the day’s events


• Teaches and assists all residents in areas of rehab including nutrition and healthy living, meal preparation, hygiene, laundry, banking and budgeting, and basic home care skills.


• Assists/Manages administrative functions and tasks at the direction of the supervisor


• Prepares and facilitates educational and skill building groups and one to one intervention as directed by the treatment plan.


• Collaborates and coordinate care with providers and families as needed.


• Assists residents in addressing medical and psychiatric needs.


• Maintains documentation in compliance with agency and program standards.


• Participates in staff meetings, supervision, trainings and other meetings as assigned.


• Additional duties as required.

COMPENSATION*:

$19.28/hour min- $27.9863/hour max

*Actual rates within this range are determined at the time of offer and are based on relevant experience, as well as adherence to our internal equity policy. (Please note, for part-time positions this range would be pro-rated based on hours worked)
