Responsibilities

• Evaluate residents' needs to identify services that enhance their quality of life.
• Establish and maintain connections with local service providers.
• Create and sustain educational health and wellness programs.
• Develop a directory of community resources for residents.
• Act as an advocate for residents, empowering them to advocate for themselves.
• Build relationships with community agencies and networking services.
• Ensure effective communication between residents and service providers.
• Advocate with management and external agencies providing social services.
• Provide crisis intervention support for residents in need.
• Track and monitor resident referrals to external agencies.
• Maintain accurate records, including daily and monthly reports.
• Perform additional tasks as assigned to support RHF's mission.
