Job summary

Outfield-Healthcare-Partners in Roswell, NM seeks a Recreation Therapist to plan, organize, and implement a comprehensive activity program addressing social, spiritual, intellectual and physical needs of residents according to their care plans.

The role includes assessing resident preferences, coordinating calendars, facilitating group and individual activities, and contributing to care plan meetings to promote engagement and well-being.

Responsibilities

• Develops and implements comprehensive therapeutic recreation programs to meet psycho-social, physical, and cognitive needs.
• Performs the assessing, selecting, organizing and implementation of recreational services to meet individualized needs.
• Develops and delivers activity/recreational programming to promote resident engagement and maintenance of functioning.
• Completes the “Preferences for Routine and Activities” section of the MDS, attends Care Plan Meetings, and reviews progress regularly.
• Conducts admissions interviews and documents activity sections of the MDS assessment tool and related care plans.
• Completes a monthly calendar of activity/recreational programs and posts it in the center.
• Identifies activity preferences and health issues and documents findings with sources when applicable.
• Charts attendance and participation daily; monitors risk factors and changes in condition.
• Plans community activities that involve community, staff, families and residents.
• Provides residents opportunities for group and individual leisure pursuits and invites participation.
• Supports residents’ right to self-determination and documents refusals of offered services.
• Assists residents in organizing Resident Council Meetings and coordinates an effective Volunteer Program.
• Participates in continuing education programs and in-service training for department personnel.
• Participates in department budget planning and reports monthly on activities supplies; stays within budget.
• Other duties may be assigned with or without notice.
