Job summary

Qualifications

• High school diploma or equivalent preferred.
• Must be a registered CNA by the State.
• Active Illinois Driver's License required.
• Class C license with passenger endorsement for vehicles which transport 15+ passengers.
• Class E license for vehicles that transport 14 or fewer passengers.
• No driver under 25 or over 74 per insurance requirements.
• DOT compliant physical.
• Experience as a bus/van driver preferred.
• Ability to communicate effectively in writing and speaking.

Responsibilities

• Oversees restorative programs for individual patients.
• Communicates with Nursing Director about goals and approaches.
• Facilitates transition from skilled rehab to restorative programs.
• Attends rehab rounds and reports patient status.
• Maintains restorative progress notes for all patients.
• Visits patients daily to ensure program suitability.
• Trains caregivers in restorative techniques.
• Monitors ROM and T&P orders for compliance.
• Instructs on splints, braces, prosthetic appliances, and self-help devices.
• Directs and participates in restorative programs.
• Assesses new admissions for restorative needs and informs staff.
• Updates families on patient progress during care planning.
