Requirements include a bachelor’s degree in social work or related field, one year of supervised social work experience in a health care setting, and licensure.

Responsibilities

• Oversee development, implementation, supervision, and evaluation of the Social Services Department to meet residents' well-being.
• Facilitate residents’ safe transition back into the community based on wishes, safety, and clinical recommendations.
• Collaborate with the interdisciplinary team to identify discharge planning needs and arrange community-based services.
