Adept of Omaha is seeking a compassionate Social Services Coordinator to support residents and families, under the Social Services Director. The role focuses on addressing social, emotional, and psychosocial needs while promoting dignity and quality of life.

Responsibilities include assessments, care planning, admissions/discharges, and coordinating community resources with timely documentation. Excellent communication with residents, families, and the interdisciplinary team is essential.

Formación

• Two years’ experience in a social worker role, preferably in a health care setting.

Responsabilidades

• Assist with resident assessments and psychosocial assessments.
• Maintain accurate social services documentation within required timelines.
• Orient residents and families to the facility and available services.
• Participate in care conferences and interdisciplinary meetings to address psychosocial needs.
• Assist with admissions, transfers, and discharge planning with families and staff.
• Coordinate admission paperwork and inventories and ensure proper labeling.
