Job summary

Adept of Omaha is seeking a compassionate Social Services Coordinator to support residents and their families under the Social Services Director. You will assist with assessments, care planning, admissions, discharge coordination, and access to community resources.

Qualifications

• Two years’ experience in a social worker role, preferably in a health care setting.

Responsibilities

• Gather social histories and complete psychosocial assessments under the direction of the Social Services Director.
• Maintain accurate, current social services documentation within required facility and regulatory timelines.
• Orient residents and families to the facility, available services, service limitations, and residents’ rights.
• Participate in care conferences and interdisciplinary meetings, communicating residents’ psychosocial needs, preferences, and concerns.
• Assist with scheduling care conferences and notifying residents and families of meeting times, changes, or cancellations.
• Coordinate the completion of admission paperwork and resident belongings inventories, including labeling and documentation.
