Job summary

Peabody Retirement Community in Indiana is seeking a compassionate Social Services Coordinator to support residents’ psychosocial needs within our Skilled Nursing team. You will work closely with residents, families, nursing, and the interdisciplinary care team to plan care, host meetings, and coordinate resources, discharge planning, and relocation support.

Qualifications

• Experience in social services or resident services in long-term care.
• Strong communication, organization, documentation, and interpersonal skills.
• Background in social work, psychology, gerontology, nursing, or related field preferred.

Responsibilities

• Assist with care planning and resident/family meetings.
• Support MDS documentation and discharge/relocation planning.
• Address behavioral health needs and coordinate community resources.
• Help residents and families navigate the long-term care experience.
