Job summary

Qualifications

• Two or more years of social service experience preferred.

Responsibilities

• Coordinate programs and activities for eligible individuals.
• Serve as liaison to community agencies and arrange services.
• Document services and maintain participant records and databases.
• Develop and maintain a Resource Directory and organize educational events.
• Ensure care plans and monitoring with follow-up and reporting.
