Job summary

Qualifications

• 2 years’ experience in case management and experience in homeless programs required.
• AA degree or higher in human services, social work or psychology required.
• Some experience in community outreach and available community services highly desirable.

Responsibilities

• Maintain a calm, friendly environment within the facility and with participants.
• Enter client data into HMIS for the location.
• Conduct intakes and assessments for permanent supportive housing.
• Coordinate with HUD VASH case managers for tenants’ programs/referrals.
• Maintain logs of maintenance issues and report to Site Director.
• Develop an Individual Treatment Plan (ITP) or ISP with the tenant.
