Responsibilities

• Collaborate with patients/residents and their families to provide support and resource information.
• Respond to issues identified by patients/residents and their families.
• Facilitate patient/resident transfer within the nursing center to ensure seamless transition and document accordingly.
• Participate in healthcare decision-making within the nursing center under the direction of a qualified social worker or consultant.
