Essential Duties and Responsibilities
Resident Support & Advocacy
  • Serve as a connector, coordinator, educator, advocate, and documenter for residents while maintaining clear professional boundaries.
  • Maintain a visible, proactive presence with clear office hours and ongoing professional relationships with residents.
  • Conduct resident intake and non-clinical assessments to identify priorities, risks, strengths, activities of daily living, and service needs without diagnosing.
  • Develop or update resident services plans collaboratively with participating residents; respect and document a resident’s decision to decline participation.
  • Provide general or limited case management when appropriate and when the needed service is not otherwise available through the larger service community.
  • Assist residents in accessing public benefits and supportive resources, including food, health coverage, income supports, energy assistance, transportation, and other locally available services.
  • Make referrals with resident permission, obtain signed consent before sharing protected information, and follow up to determine whether services were timely, appropriate, and responsive to the resident’s need.
  • Advocate and, when appropriate, negotiate on behalf of residents for adequate, timely, and cost-effective services while avoiding unhealthy dependence.
Program Development and Community Partnerships
  • Develop and coordinate educational, wellness, prevention, employment-readiness, and community-engagement activities based on identified resident needs.
  • Coordinate on-site services, screenings, clinics, workshops, speakers, volunteer opportunities, resident meetings, resource fairs, and community-building activities.
  • Build productive relationships with local, state, and federal agencies, nonprofit organizations, service providers, and volunteer groups.
  • Maintain a current resource directory that identifies who each provider serves, referral procedures, consent requirements, follow-up steps, and options when access barriers arise; update it at least annually and whenever provider information changes.
  • Identify service gaps, pursue additional resources, and elevate barriers that limit resident access or program effectiveness.
  • Strengthen informal support networks among residents and with families, as appropriate and with resident consent.
Documentation, Privacy and Reporting
  • Maintain complete, objective, prompt, organized, and secure resident case files consistent with HUD requirements and company policy.
  • Document intake information, assessments, services plans, resident contacts, referrals, consent, participation or refusal, follow-up, progress notes, and disposition or termination of services in the approved case-management system.
  • Maintain program records, including training documentation, reports, provider agreements, program files, eligible expense records, and other required supporting documentation.
  • If applicable, prepare accurate internal and HUD-required reports, including the HUD Multifamily Housing Service Coordinator Program Annual Report (HUD Form 92456).
  • Protect resident privacy and share resident-specific information only when authorized, necessary, and consistent with HUD, HIPAA, Fair Housing, organizational confidentiality, consent, and records-security requirements.
  • Take immediate action to correct missing, late, inaccurate, or insecure documentation and participate in file reviews and quality-assurance activities.
Communication, Collaboration and Escalation
  • Maintain strong communication and collaboration with property staff, supervisors, service providers, and quality assurance partners to address resident needs, service gaps, risks, and housing stability concerns.
  • Work closely with property management while appropriately referring leasing, rent collection, maintenance, and enforcement matters to their team, and coordinating interventions for residents at risk of eviction or housing instability.
  • Promptly escalation safety, abuse, neglect, or legal concerns according to protocol, while meeting all training, reporting, documentation, and professional responsiveness requirements.
Knowledge, Skills and Abilities
  • Complete at least 36 hours of initial training before hiring or within 12 months of the initial hire date. Topics include aging, federal and state programs, elder and disability services, legal liability, substance use, and mental health.
  • Knowledge of HUD Multifamily housing programs, Section 8, service coordinator funding requirements, Fair Housing, reasonable accommodations, and confidentiality obligations.
