• Works as part of an interdisciplinary team to support residents' activities, social, emotional, and economic needs.
• Develops care plans and facilitates care conferences.
• Investigates grievances and completes assessments.
• Provides knowledge of teamwork and communication with residents, families and the healthcare team.
