Job summary

Arden Park Post Acute is seeking a dedicated Physical Therapist to evaluate and treat residents in a post-acute care setting. You will collaborate with an interdisciplinary team to enhance mobility and independence and document progress accurately for ongoing care planning.

Qualifications

• Valid California Physical Therapist license
• Experience in skilled nursing or post-acute care preferred
• Strong communication and documentation skills

Responsibilities

• Evaluate residents to determine physical therapy needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Supervise Physical Therapy Assistants and coordinate with the interdisciplinary team.
• Communicate with physicians, nursing staff, residents and families regarding progress and discharge planning.
• Maintain accurate documentation and ensure compliance with policies and third-party payers.
