Qualifications

• Minimum of 3 years of social work experience in a healthcare setting, preferably in senior settings.

Responsibilities

• Responding to residents in need of psychosocial or supportive services, through direct referral or by review of incident reports
• Conducting assessments of potential residents
• Recommending appropriate levels of residency
• Assessing and resolving issues regarding residents' care and safety
• Developing referral networks within the community for available services
• Educating residents in the importance of Advanced Directives and completion of documents
