Job summary

Center for Disability Services in the Capital Region is seeking a Resident Counselor. Openings exist at multiple locations; rate of pay depends on experience, acuity, and trainings completed.

Flexible FT/PT/Relief in a 24/7 setting with weekend options and shift differentials. Responsibilities include participating in planning and interdisciplinary teams, supporting independence in daily care, and implementing individualized habilitation plans to help people we support achieve their goals.

Qualifications

• High School diploma or equivalent.
• One year of related experience preferred.
• Must be at least 18 years old and able to lift 50 pounds.

Responsibilities

• Serve as a member of the person’s planning, implementation, and interdisciplinary teams.
• Assist, promote, and provide independence in daily treatment, care, and services; respect choices and dignity.
• Help implement personalized habilitation services per care plan.
• Assist in acquisition of skills, including daily living activities and household management.

Education

One year of related experience preferred

Responsibilities

• Serve as a member of the person’s planning, implementation, and interdisciplinary teams


• Assisting, promoting, and providing independence in the daily treatment, care, and services of the people we support. Respect the person’s choices, lifestyle, and their dignity to take informed risks


• Help implement personalized habilitation services, which are outlined in their personalized care plan


• Assist in acquisition of skills, including basic activities of daily living, self-care and household management for the people we support

Requirements

• High School diploma or equivalent


• One (1) year of related experience preferred


• Must be at least 18 years old and able to lift a minimum of 50 pounds to apply for this position

Compensation Range
