Job summary

Qualifications

• High school diploma or equivalent is required.
• Preferable one-year experience in a long-term care facility.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct resident-centered activities.
• Keep up with federal/state regulations and professional standards.
• Communicate effectively with staff, residents, families, and agencies.
• Participate in community planning for facility interests and services.
• Participate in facility surveys conducted by government agencies.
• Assist in reviewing and developing plans of correction for deficiencies noted during surveys.
• Attend regular meetings (staff, department head, QA, teams, safety, marketing, family and resident council).
• Assist QA/Assessment Committee in action plans to correct quality deficiencies.
• Participate in discharge planning and activity care plans for residents.
• Arrange transportation for outings, appointments, and discharges.
• Develop monthly activity schedules including outings and group/in-room activities.
• Encourage resident participation in self-initiated activities and provide reading materials (Braille/audio).
• Ensure progress notes are informative and descriptive of services and resident responses.
