Job summary

Manchester-Center-For-Rehabilitation-and-Healing-1 is seeking a Social Worker to join its nursing home team in Nashville. The role focuses on improving residents’ ability to manage daily physical and psychosocial needs, identifying social and emotional needs, and developing individualized care plans.

The position involves coordinating admissions, transfers, discharge planning, and ensuring proper documentation with the finance team. PCC experience is preferred and a flexible schedule is required.

Qualifications

• BSW or MSW preferred, or Bachelor's degree in the related field.
• At least 3 years of experience in the Social Work field.
• Ability to work a flexible schedule.
• PCC experience preferred.

Responsibilities

• Maintaining and/or improving the resident's ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident's admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Education

BSW or MSW or Bachelor's degree in related field

PCC experience preferred
