This role participates in care planning and discharge planning, reporting changes in psychosocial functioning with the care team.

The position requires a Bachelor’s degree in a related human services field and at least 1 year of supervised social work experience in healthcare.

Qualifications

• Bachelor’s degree in a related Human Services field or equivalent combination of education and experience.
• 1+ years of supervised social work experience in a health care setting.

Responsibilities

• Identify medically-related social service needs of residents and coordinate appropriate referrals.
• Maintain contact with families (with resident’s permission) to report changes and support care planning.
• Assist residents with financial and legal matters such as guardianship or pre-paid funeral arrangements.
• Coordinate discharge planning services and home care arrangements as residents transition.
• Support residents’ individual needs and preferences through assessment and care planning.
