Beaverton Post Acute of Cascadia is seeking a confident Social Services Manager to advocate for residents and lead social services staff. The role emphasizes planning, coordinating, and delivering resident-centered social programs while ensuring regulatory compliance and collaboration with community partners.

Ideal candidates will bring leadership, a background in social work, and experience in post-acute or skilled nursing settings to support comprehensive care and successful discharge planning.

Qualifications

• Bachelor’s or Master’s degree in social work preferred.
• LCSW, LSW, or MSW license preferred but not required.
• Must have a valid driver’s license.
• Experience in post-acute/ skilled nursing preferred.

Responsibilities

• Assess psychosocial status and communicate findings to the nurse.
• Plan and implement person-centered social interventions.
• Maintain clinical documentation for all social work referrals.
• Coordinate referral services for residents and families.
• Inform residents and families about community services available.
• Liaise with community agencies and residents/families.
• Collaborate with facility staff to support care planning.
• Participate in care planning and case conferences as needed.
• Assist team in understanding social factors affecting health.
• Establish behavior monitoring to support quality care.
• Address resident behaviors to prevent harm.
• Support discharge planning and post-discharge needs.
• Coordinate medication review for gradual dose reduction.
