Job summary

PruittHealth is seeking a Social Services Director I to coordinate and direct social services per federal, state, and local regulations. The role involves advocating for residents’ rights, coordinating psychosocial assessments, discharge planning, and handling grievances within a long-term care setting.

The position requires a bachelor’s degree in Social Work or a related field, with at least one year of supervised healthcare social work experience; MSW is preferred.

Qualifications

• Minimum of one year of supervised social work experience in healthcare or long term care setting.
• Previous supervisory experience.
• MSW preferred.

Responsibilities

• Coordinate and direct Social Services in accordance with federal, state and local regulations and guidelines.
• Respect the rights, dignity and individuality of each resident in all interactions.
• Advocate for residents’ rights and coordinate behavioral health referrals as needed.
• Complete psychosocial assessments and provide treatment recommendations.
• Coordinate discharge plans with residents and families.
• Assist patients and families with advance directives under Federal regulations.
• Resolve and track grievances.
• Complete psychosocial assessments of residents.
• Coordinate family council meetings.
• Coordinate ancillary appointments as necessary.

Education

Bachelor’s degree in Social Work or Human Services
