Job summary

Yardley Rehabilitation and Healthcare Center is seeking a Social Services Director to lead the social services program, addressing residents’ emotional and practical needs and guiding families through transitions.

You will collaborate with interdisciplinary teams, ensure regulatory compliance, provide advocacy and counseling, coordinate discharge planning, maintain records, and mentor staff. A Bachelor’s degree is required; a Master’s is strongly preferred and licensure (LSW/CSW) is required.

Qualifications

• Bachelor’s degree required; Master’s degree strongly preferred.
• Current LSW/CSW licensure required.
• Experience as a social worker in a healthcare setting is required.
• Experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations governing nursing care facilities.
• Excellent communication skills with residents, families, staff, and agencies.
• Ability to balance advocacy with empathy, professionalism and emotional support.

Responsibilities

• Provide leadership and oversight of social services programs for residents and families.
• Ensure services comply with federal, state, and facility regulations and standards.
• Collaborate with interdisciplinary teams to support resident-centered care planning.
• Provide counseling, support, and advocacy during admission and transitions.
• Serve as liaison between residents, families, staff and external agencies.
