Qualifications

Responsibilities

• Assist in identifying and addressing residents’ psychosocial, emotional, and mental health needs.
• Complete social histories, psychosocial assessments, and required documentation.
• Participate in interdisciplinary care planning and resident care conferences.
• Support resident rights, advocacy, and grievance resolution.
• Assist with PASRR processes, advance directives, and resident rights education.
• Provide support to residents and families during admissions, room changes, and transitions.
• Coordinate discharge planning activities and community resource referrals.
• Assist with Medicaid applications and benefit coordination as directed.
• Document progress notes and resident interactions accurately and timely.
• Support surveys, audits, and regulatory compliance efforts.
• Participate in quality assurance and performance improvement activities.
• Maintain confidentiality and comply with HIPAA requirements.
