Job summary

Kern River Transitional Care is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. This role focuses on evaluating residents, developing individualized treatment plans, and coordinating with families and physicians to improve mobility and independence.

The position requires California licensure, strong documentation, and collaboration with an interdisciplinary team to ensure safe, effective care and ongoing resident education.

Qualifications

• Bachelor’s degree in Physical Therapy required.
• Master’s or Doctorate degree in Physical Therapy preferred.
• Proficiency with Net Health and PointClickCare computer systems.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Assist nursing with training Restorative Aides.
• Supervise Physical Therapy Assistants in direct resident care.
• Communicate progress and goals with physicians, nursing staff, residents and families.
• Participate in Resident Care Conferences and Rehabilitation meetings.
• Provide in-services and training for facility staff as needed.
• Complete timely, accurate documentation per policies.
• Educate residents and families on home programs and safety.
