Job summary

Qualifications

• Bachelor's degree in Physical Therapy; Masters or Doctorate preferred.
• Licensed as a Physical Therapist in the state; must meet CE requirements.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and functional independence.
• Supervise PT assistants and training for Restorative Aides as applicable.
• Communicate with physicians, nursing staff, and families regarding progress and discharge planning.
• Complete timely, accurate documentation in accordance with policies and payer requirements.
• Maintain infection control and safe treatment environment.
