Job summary

Pikes Peak Post Acute seeks a skilled Staff Physical Therapist to evaluate residents, develop effective treatment plans, and supervise PT assistants. The role involves collaboration with physicians, nursing staff, residents, and families to advance mobility and independence.

The incumbent will maintain compliant documentation, participate in training initiatives, and ensure infection control standards are met while supporting quality rehabilitation programs.

Qualifications

• Licensed Physical Therapist in the state (Oregon).
• Must meet continuing education requirements and maintain license.
• CPR certification if required by licensure.

Responsibilities

• Evaluate residents to determine PT needs per physician orders.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and aides in resident care.
• Communicate with physicians, nursing staff, residents and families about progress and discharge plans.
• Complete timely documentation in compliance with regulations and payers.
