Job summary

The Legacy at Boca Raton Rehabilitation and Nursing Center is looking for a dedicated Physical Therapist to join our rehabilitation team in Boca Raton, FL. You will evaluate residents, create individualized treatment plans, and provide high-quality therapy to improve independence and functional mobility.

Ideal candidates will have an active Florida PT license, strong clinical assessment abilities, and a compassionate, team-oriented approach to resident-centered care.

Qualifications

• Active Florida Physical Therapist (PT) license in good standing.
• Previous experience in a skilled nursing facility, rehabilitation center, or healthcare setting preferred.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively within an interdisciplinary team.
• Compassionate and resident-focused approach to care.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans
• Provide therapeutic exercises, mobility training, balance training, and pain management interventions
• Monitor resident progress and adjust treatment plans as needed
• Collaborate with physicians, nursing staff, and interdisciplinary team members to ensure optimal resident outcomes
• Maintain accurate and timely clinical documentation in accordance with regulatory and facility requirements
• Educate residents and families regarding therapy programs and treatment goals
• Ensure compliance with all federal, state, and facility policies and regulations
• Participate in care planning meetings and discharge planning processes
• Maintain a safe and supportive therapy environment
