The Legacy at Boca Raton Rehabilitation and Nursing Center is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team in Boca Raton, FL. Full-Time or Part-Time opportunities are available, with a competitive compensation package.

The ideal candidate will evaluate residents, create individualized treatment plans, and deliver high-quality therapy to help residents achieve maximum independence and mobility.

Formación

• Florida PT license in good standing is required.
• Experience in skilled nursing facility or rehabilitation setting preferred.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and collaboration with interdisciplinary team.

Responsabilidades

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide therapeutic exercises, mobility, balance training, and pain management interventions.
• Monitor resident progress and adjust plans accordingly.
• Collaborate with physicians, nursing staff, and therapists to optimize outcomes.
• Maintain accurate, timely documentation per regulatory and facility requirements.
• Educate residents and families on therapy goals and progress.
• Participate in care planning and discharge planning processes.
