The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL is seeking a dedicated Physical Therapist (PT) to evaluate residents, develop individualized treatment plans, and provide high-quality therapy to help residents achieve their highest level of independence and functional mobility.

Qualifications

• Active Florida Physical Therapist license in good standing.
• Experience in skilled nursing facility, rehabilitation center, or healthcare setting preferred.
• Strong clinical assessment and treatment planning skills.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide therapeutic exercises, mobility training, balance training, and pain management interventions.
• Monitor resident progress and adjust treatment plans as needed.
• Collaborate with physicians, nursing staff, and interdisciplinary team members to ensure optimal resident outcomes.
• Maintain accurate and timely clinical documentation in accordance with regulatory and facility requirements.
• Educate residents and families regarding therapy programs and treatment goals.
