Job summary

Alpha Home Health and Hospice in Escanaba, MI seeks a dedicated Licensed Practical Nurse (LPN) to provide compassionate, resident-centered care in a skilled nursing and rehabilitation setting.

You will deliver bedside nursing, administer medications, monitor vital signs, and document in the EMR, while collaborating with RNs and the interdisciplinary team to support rehabilitation plans and enhance comfort, function, and quality of life for residents.

Qualifications

Responsibilities

• Assist with rehabilitation and long-term care plans under RN and provider direction.
• Collaborate with an interdisciplinary team to ensure coordinated care.
• Educate residents and families and support care understanding.
• Maintain safety and infection control standards.
