Job summary

Quality Center for Rehabilitation and Healing is seeking a compassionate Licensed Practical Nurse (LPN) to provide quality care to residents. You will work with the nursing team to support residents’ health, comfort, and daily care needs in accordance with care plans and applicable requirements.

Responsibilities include administering medications, monitoring health, documenting care, and communicating with residents, families, and care team members.

Qualifications

• Current LPN license in the state where the facility is located.
• Strong communication and interpersonal skills.
• Compassionate, respectful approach to resident care.
• Ability to work collaboratively with the nursing and care teams.
• Long-term care or rehabilitation experience preferred.

Responsibilities

• Provide nursing care to residents within the scope of the LPN license.
• Monitor residents’ health and report changes to nursing staff.
• Administer medications and treatments as assigned and per care plans and facility policies.
• Document care provided and maintain resident records.
• Communicate with residents, families, and care team members.
• Support residents with daily care needs as appropriate.
• Follow infection prevention, safety, and confidentiality practices.
