Job summary

A leading senior living organization located in New Providence is looking for a Social Worker to deliver high-quality social work services to residents and their families. The role requires a Social Worker degree, ideally with an MSW and 3+ years of experience in post-acute care, with a focus on geriatrics. Responsibilities include psychosocial assessments, care planning, and providing bereavement support. Qualifications

• Minimum of 3 years of experience in post-acute/long-term care.
• Knowledge of geriatrics and dementia is essential.

Responsibilities

• Complete psychosocial assessments and required documentation.
• Coordinate the Social Work function from admission through discharge.
• Participate in the care plan process with interdisciplinary teams.
• Provide bereavement support to residents and families.
• Monitor psychosocial and medically related social service needs.
• Ensure compliance with regulatory standards.
